Nursing
Skilled nursing includes services such as disease assessment, medication management, wound care, and teaching patients and families to care for themselves. Visiting nurses work closely with patients' physicians to ensure continuity of care.
Hospice
Hospice cares for terminally ill patients and their families. The program offers a medically supervised, nurse coordinated team who provide physical, emotional and spiritual comfort throughout the illness and bereavement experience. Pain and symptom management are of primary importance.
Physical, Speech and Occupational Therapies
Rehabilitation services help patients overcome physical and communication barriers caused by stroke, injury or other illness.
Specialty Nursing
Registered nurses provide expertise and patient education in the areas of diabetes management, palliative care, and wound care.
Social work Services
Medical social workers help patients and families adjust to illness or injury, and provide information on available resources within the community
Home Health Aides
Certified staff assist patients with daily tasks including personal hygiene, ambulation and light meal preparation.
Maternal/Child Health
Specially trained nurses provide care for pregnant women and new mothers and their newborn infants.
Personal Response Systems: Constant Companion
The program provides support to those that need to easily summon help in an emergency with the push of a button.
Patient/Family Wellness
Trained staff provides influenza immunizations and injury prevention
programs such as falls prevention.
